I , hereby authorize the release of my employment and personal information to my CareerLink Employment Specialist. This information will be used solely for the purpose of helping me find employment, including identifying job opportunities, assisting with resume development, preparing for interviews, and providing guidance on job applications.

Consent for Sharing with Employers:

I understand that the employment specialist may share employment related information with potential employers, staffing agencies, or related entities as necessary to assist with my job search. This consent includes, but is not limited to, providing my resume, employment history, and skills to employers or job placement organizations.

In granting such permission, I understand that such information will remain part of my confidential case file and that such information will be used to give the best available professional help. In addition, I release the staff of HopeLink from any liability arising from the release of the information to any agency/service provider.

Acknowledgment and Consent:

I understand that I am voluntarily providing consent for the release of my information and that I may revoke this consent at any time by notifying the employment specialist in writing. This authorization is valid for the duration of my job search, unless revoked earlier. I may request to review or update the information that has been shared at any time. I understand that the information provided will be used solely for the purpose of finding employment and will not be shared beyond that scope without my further consent.
